Included in the registration fee are daily breakfast, lunch, and coffee breaks as well as admission into the Keynote presentation.

For U.S. registrants: The Central Ticket Office will mail a confirmation sheet as proof of your conference registration, to the address that you provide on the registration form. This will be sent within one month of registration. Please keep your credit card statement (which will read, "UCLA Tickets 8252101") or cancelled check as it will be your only proof of registration prior to receiving the confirmation sheet in the U.S. mail. Please bring your confirmation sheet to the conference registration desk when you sign in on the day of the conference to get your conference package.

FPR-UCLA GRADUATE DIVISION SCHOLARSHIPS FOR UCLA STUDENTS

All other students:
All other students can only register through mail or by Fax (no on-line registration for students). To be eligible for the students' fee, a copy of student ID or proof of student status is required, to be enclosed or faxed together with the completed registration form.
International students without a credit card may register by mail. You will need to send to UCLA Central Ticket Office (see above for mailing address), the following:
- completed hard-copy registration form
- a money order for student registration fee, drawn on U.S. funds (United States dollar), payable to: UC Regents, and
- a copy of your student identification or proof of student status.
Write on your registration form, "WILL CALL," which means that the conference ticket will be available for you to pick up at UCLA on the day of the event (it will not be mailed to you).
ON THE DAY OF THE CONFERENCE:
Please bring your proof of registration to the conference registration desk at the Korn Convocation Hall on February 11, 2005 morning to sign up and get your conference program package (program booklet, name badge, a Keynote presentation ticket, and information on the meals, etc.). Your name badge is required for admission to the conference hall and its related venues. Students: please bring your photo student identification card when you sign in.
CONTINUING MEDICAL EDUCATION:
This is a Category 1 Continuing Medical Education (CME) activity offered by the UCLA Neuropsychiatric Institute, a CMA-accredited provider. Physicians and Psychologists attending this course may report up to 20 hours of Category I credits toward the California Medical Association's Certification in Continuing Medical Education and the American Medical Association's Physician's Recognition Award. Each Physician or Psychologist should claim only those hours of credit that he/she actually spent on the educational activity.
If you apply for CME credits, you must provide your license number on the daily sign-in sheets at the CME registration desk on the day of the conference.
